Machine amputated worker's fingers
A malfunctioning Endoline tape packaging machine removed 3 fingers from a worker who was attempting to repair it in June last year.
The machine started up suddenly, a safe isolation procedure was not available.
Belcher Food products Ltd of Prestwick was fined £30,000 for what an HSE official called "an entirely foreseeable and preventable accident."
Three months before the accident, HSE inspectors told the company they must have a procedure in place to ensure machinery could be properly isolated while being repaired, but this advice was not acted upon.
